You pick the amount of coverage you need (usually roughly what your deductible or out-of-pocket is on your main health plan) and then which services you want to cover (accidents, hospitalization, critical illness, or all three). The plan is backed by Allstate (yes, that Allstate), and coverage gets slightly better each year you keep the plan.
